Terms of Service

These terms govern your use of Butler, the care navigation service operated by Parlor Health, Inc. By enrolling, you agree to them.
Butler is not for emergencies. If you are having a medical emergency, call 911 or go to the nearest emergency room. We do not monitor messages around the clock and we cannot respond to urgent problems.
What we do
Parlor Health provides care navigation between your office visits, under the name Butler. Our navigators check in on your condition, help you get to appointments and services you need, and bring concerns to the attention of your clinician.
What we do not do
- We do not diagnose conditions or prescribe medication.
- We do not change instructions your clinician has given you. If you think something should change, we will bring it to them.
- We do not provide emergency or after-hours care.
- We do not replace your relationship with your other providers.
Messaging
With your consent, we communicate by text message and phone. Messages are part of your medical record. Please do not use them for anything urgent. Reply STOP at any time to stop text messages; see our Privacy Policy for details. Standard message and data rates may apply.
Cost and billing
Our services are billed to Medicare under the Principal Illness Navigation and Community Health Integration benefits.
- Cost sharing applies. You may owe a share of the cost, depending on your Medicare coverage and any supplemental plan you hold. We will tell you before services begin.
- Only one practitioner may bill Medicare for these services in a given calendar month. If another practice is already providing them, tell us.
- You may stop the service at any time, for any reason, with no effect on your other care or benefits.
Your part
So that we can help you, please give us accurate information, tell us when your contact details or medications change, and let us know if you start receiving similar services elsewhere.
Consent and withdrawal
We obtain your consent before beginning services and record it in your chart. You may withdraw it at any time by telling any member of our team. Withdrawal takes effect immediately and does not affect care you have already received.
Privacy
Your health information is handled as described in our Privacy Policy and our Notice of Privacy Practices.
Availability
We aim to keep our service running, but we do not guarantee it will be available at all times. Maintenance, outages, and carrier problems can interrupt messaging.
Limits
Nothing in these terms limits our liability for our own professional negligence, and nothing here is intended to waive any right you have under federal or state law that cannot be waived.
Changes
We may update these terms. The current version will always be posted here with its date. If a change materially affects your care or what you may owe, we will tell you directly.
